Applicants will be selected at the sole discretion of Orlando Entertainments Limited whose decision will be the final.

You must be resident in the UK or Republic of Ireland. You must be AT LEAST 15 YEARS OLD ON 1ST APRIL 2008.

Your application will only be considered if this form is properly signed and dated. If you are under 18 years old this application form must be also signed by your parent or legal guardian.

At personal details and/or information given in your application either in this form or otherwise must be truthful, accurate and in no way misleading.

This is an application for the competition included as part of the television entertainment talent show provisionally entitled “Derana Dream Star London Chapter” Series 01 which the Company proposes but does not undertake to produce for Derana TV. The information given by you in this form may be used in the Programme should you be selected.

The Company reserves the right to disqualify you  if you have given untruthful, inaccurate or misleading personal details and/or information, have failed to abide by the Rules and/or are in breach of the terms hereof.

You acknowledge and agree to abide by the rules regulations terms and conditions for this competition and that the Rules may be changed at any time by the Company and that you will be bound by such Rules. The Company’s decision shall be final.

You will not, without the Company’s prior written consent, publicise or discuss with any 3rd party any information relating to the application, the competition, your appearance, the production team or any participants in the contest nor any other information which may come to your attention (including without limitation in relation to the broadcaster) Derana.

Contestants are responsible for all incidental expenses incurred in connection with the competition i.e dress, jewellery, travelling etc.
